Disinfection capabilities of a 222 nm wavelength ultraviolet lighting device

The aim of this research is to demonstrate the efficacy of the SafeZone 222 nm UVC light to reduce bacterial burden in pressure ulcers (PUs) in human patients. This research is the first human clinical trial using 222 nm UVC in eradicating bacteria in human wounds.

Patients with Stage 2 or 3 (as defined by the revised National Pressure Ulcer Advisory Panel Pressure Injury Staging System) sacral or gluteal pressure ulcers (PUs) were subjected to four sessions of 222 nm UVC light therapy over two weeks. Pre- and post-UVC therapy, wound cultures were taken and quantitative analysis of bacterial colony forming units (CFU) were performed.

The results showed that a total of 68 UV light sessions across 16 different patients were conducted. Of these sessions, 59 (87.0%) sessions showed a reduction in CFU counts, with 20 (29.4%) showing complete eradication of bacteria. Bacteria identified included meticillin-resistant Staphylococcus aureus, Pseudomonas aeruginosa and Klebsiella Pneumoniae. The overall median reduction in CFU of the 68 sessions was 78.9%. No adverse events were reported in any of the UV sessions.

In this study, the results proved that 222 nm UVC light was safe and effective in reducing bacterial CFU counts in sacral and gluteal PUs across numerous different species of bacteria.

There is a growing threat of multidrug resistant (MDR) pathogens which are both costly and difficult to treat. Studies have proved that ultraviolet (UV) light is effective in bacterial disinfection of surfaces. More specifically, it is the shorter wavelength UVC light that kills bacteria. UVC light works by penetrating the bacterial nucleus and mutating bacterial DNA which then limits bacterial replication capabilities. UVC light covers wavelengths from 200–280 nanometres (nm).

UVC light of 254 nm has been demonstrated to be effective in reducing infections from both drug-sensitive and MDR bacteria. Since 1936, 254 nm UVC disinfection systems have been used in healthcare settings, and studies conducted at multiple hospitals have successfully used overhead UVC (254 nm) disinfection systems in operating rooms, boasting a net average reduction of 80% in infection cases. Thai et al. also presented a case series in 2002 where 254 nm UVC treatment was found to reduce bacterial load and facilitate healing in three patients with hard-to-heal wounds, locally infected with meticillin-resistant Staphylococcus aureus (MRSA), in whom previous standard wound care, and topical and oral antimicrobial therapy had failed. However, widespread use of conventional 254 nm UVC lamps has been limited due to its carcinogenic and cataractogenic properties.

Studies have shown shorter wavelengths of UVC, 207–222 nm, to be equally bactericidal while being safe for use on mammalian skin. Due to the shorter penetration range of shorter wavelengths of UVC light, the light is unable to transverse the outer stratum corneum of the skin or the outer tear layer of the eye, making it safer with regards to the deeper layers of living cells. As bacteria are much smaller in dimension, the UVC light retains its germicidal properties as it is still able to penetrate the bacterial cell nucleus. Narita et al. compared the bactericidal properties and safety of 222 nm and 254nm UVC in a mouse model of a skin wound infected with MRSA. They found that 222 nm UVC light was as effective as 254 nm UVC in reducing bacterial numbers while not inducing cyclobutane pyrimidine dimers (CPD) in skin cells of the epidermal and dermal layers, unlike 254 nm UVC which induced CPD expression in skin cells of the same layers. This indicates that 222 nm UVC is a safer and effective method of disinfecting wounds.

Current management of pressure wounds is limited to mainly conservative measures, such as offloading therapies and regular wound care. Reduction of bacterial load remains an important step in wound bed preparation to encourage wound healing in such wounds. This study aims to demonstrate the efficacy and safety of the SafeZone UVC (20w far UVC light module 222nm uv light safe for human skin, with a peak wavelength of 222 nm) in reducing bacterial load on pressure ulcers (PUs) in human patients. To our knowledge, this is the first clinical study investigating the effectiveness of the 222 nm UVC in human patients.
